Review Your Medications Service

Periodically, it is a good idea to review your Medications, Nutritionals, Vitamins and Herbal Supplements. By doing this it may be possible to eliminate duplicate or conflicting therapies or recognize adverse reactions that are related to what you are taking.
 
The answers provided will allow the pharmacist to review your medication history.
All information provided will be kept confidential.
 
Mark Drugs offers two NEW services:
Prescription or OTC medications, herbal or nutritional products, may have benefits but also may have side effects or drug interactions. Mark Drugs can help. You do not need to purchase your medications from Mark Drugs to utilize this service.
  
Simple Medical Review:
Fee: $35 - A Simple Review of medications and patient concerns to determine if they are likely associated with medication side effects. A written assessment will be provided for your physician to review.

In-Depth Medical Review:
Fee: $150/hour billed in 20 minute increments - An in-depth review of medication and medical history with a follow-up letter to the patient with an assessment and recommendations that are suitable for your physician to review.

1. Fill out our ONLINE FORM - Click to access the online form
    • Enter your information & Click the Submit button at the bottom of the online form
        
2. Send us the following information:
    • A Current Medication list with Directions / Refills / History from your Pharmacies
       for the past 6 Months. Send to Mark Drugs by email, fax, mail or hand delivery.
       Include your email & phone number. (address listed below)
 
HOW DO I SIGN UP BY MAIL? Follow Steps 1-2 below:
1. PDF Printable Form - Click to download PDF file.
     • Print PDF file and fill out your information.
     • Mail or hand deliver printed form to Mark Drugs Roselle (address listed below)

2. Send us the following Information:
    • A Current Medication list with Directions / Refills / History from your Pharmacies
      for the past 6 Months. Send to Mark Drugs by email, fax, mail or hand delivery.
      Include your email & phone number. (address listed below)

    This process will not begin until payment is received. We accept checks and credit cards.
